It is extremely important that all person's volunteering for the Millbury League fill out the required "Volunteer Application," also commonly referred to as a CORI Form. The form can be downloaded HERE.
Once you have downloaded the form, and filled it in, it can be turned into any one of the League Officers, or you can pop it into the mail at the following address:
